Patient Portal
We offer an easy-to-use, protected, secure, HIPAA compliant Patient Portal for all of our patients to use. Once you register for the Patient Portal, you will have instant access to your Labs, Wellness Plans, Handouts, Forms, Receipts, Invoices, etc. You can also communicate with us using the Secure Messaging platform within the Portal, update your Contact Info, update your Rxs, add your preferred Pharmacy, and more!
New Patients: You will need to contact our office (480-689-4200) in order to register for the portal
Existing Patients (that have already registered for the portal): Click on the below link and enter your log in information
Existing Patients (that have not registered for the portal, but want to): Contact us so we can send you a "special link" to register
New Patient Paperwork: Once you are registered for our online Patient Portal, you can add your Contact Info & fill out all of the New Patient Paperwork Forms.
Please log in and click on each of the following icons, making sure to fill out all of the information on the following pages:
- My Account
- New Patient Paperwork - 11 Forms
- My Medications
- My Supplements
- My Pharmacies
Also make sure to upload a photo of you doing something you love!
** If we do not receive your completed New Patient Paperwork at least 3-5 days prior to your scheduled Initial Consult, we may need to reschedule your consultation for a later date **
